I understand that participation in Kokum's Outreach activities is voluntary and may involve risks. I accept full responsibility for any injuries or damages that may occur and release Kokum's Outreach, its staff, facilitators and volunteers from any claims. I authorize Kokum's Outreach to seek medical care and transport me for emergency treatment if necessary. By submitting this form, I confirm I am fit to participate and agree to these terms.
